Palliative care vs. hospice: What’s the difference?
Palliative care can begin at any stage of serious illness, even while you’re still receiving treatment. Hospice is end-of-life care. While we don’t provide hospice care, we help patients and caregivers navigate that transition, if needed, with clarity and compassion.
What is included in palliative care?
Palliative care is specialized medical care that addresses the physical, emotional, nutritional, and social needs of a patient living with a serious illness or chronic condition, as well as the needs of their loved ones. Our Palliative Care includes a specialized care team that provides:
- Symptom and disease management.
- Visit summaries and collaboration with the patient’s doctors.
- Communication with loved ones and other caregivers.
- Advance care planning.
- Coordination of in-home care and support services.
- 24/7 support.
Advance care planning and care coordination
Carelon supports shared decision-making and documentation through care plans that include advance directives and goals-of-care discussions. We provide care for patients in between their appointments and treatment schedules.
Palliative care drives better outcomes
Studies show palliative care reduces hospitalizations and improves member satisfaction, helping plans manage costs more effectively.
What caregivers can expect from palliative care
Helping a loved one through a serious illness can be challenging. You don’t have to do it all alone. Learn how we support caregivers with resources, education, and emotional care.
“Palliative care is often misunderstood. It's sometimes equated to hospice care. But palliative care actually starts with the diagnosis of a serious illness. It works alongside primary care providers, specialty services, social workers, all to make it possible to live with the highest degree of independence and quality of life.”
— Liz, Carelon Nurse Practitioner
