Our palliative care team

The Carelon palliative care team is a physician-led interdisciplinary group of health care providers who help to improve the quality of life for patients experiencing serious illnesses.

At Carelon Palliative Care we manage patients’ symptoms, helping to relieve your pain, while offering emotional support to you, the patient, as well as to your family.

We dedicate ourselves to enhancing patient quality of life, because we believe that everyone deserves comfort and dignity during their most challenging times. Our whole health approach, which focuses on integrating the physical, social, and emotional aspects of patient care, makes a significant impact by easing patients' suffering and empowering families with guidance and support.

We ensure that both patients and their loved ones feel heard, understood, and cared for throughout your entire care journey.

Teamwork is the only way we work

Doctors know their patients’ health best. Led by a palliative care physician and joined by nurses, social workers, and support teams, Carelon delivers care at home to people living with serious illness, following their existing doctor's treatment plan. We work closely with patients and their caregivers, primary care providers and specialists to give full support. When it comes to helping patients live life their way, team work is the only way we work.
